New Patients
Patients new to the practice should arrive at least 15 minutes prior to their scheduled appointment time in order to complete health information forms.
Please bring all your current medications. Also, please bring a copy of your most recent medical records (1-2 Years) or more if you have a significant medical history.

In accordance with the guidelines of the insurance companies, co-payments are collected at the time of your visit; otherwise a billing surcharge may be imposed.
Payment for services not covered by insurance is due at the time of your visit. For your convenience, we accept cash, checks, MasterCard, and VISA.
